Queremos sus pensamientos e ideas honestos para guiar nuestra planificación y ayudarnos con nuestra mejora continua para proporcionar una educación de calidad para cada niño que confíe en nuestro cuidado.
Vaya al sitio web del campus a continuación para completar la encuesta de la escuela de su hijo. Si tiene varios hijos en diferentes campus, se le permitirá completar uno para cada escuela.
La encuesta tomará 15 minutos de su tiempo, pero nos proporcionará valiosos comentarios. La fecha límite para completar la encuesta es el 14 de diciembre.
On December 4th, the Tyler State Health Department will be coming to the Primary School Campus to give immunizations to any Mineola ISD student needing them. The immunizations will be given Wednesday, December 4 from 3:30 pm until 5:00 pm at the Primary School Campus at 1500 W. Loop 546.
Flu shots will also be offered during this time. The cost for the flu shot will be $25.
Below you will find the eligibility requirements for these immunizations. If you are unable to pay due to lack of insurance coverage or financial reasons, please contact Brandi Davlin at 903-569-5488 to inquire about some assistance for immunizations that are required.
The child has to qualify for one of the below eligibility status:
a.) Is enrolled in Medicaid, or is Medicaid eligible *
b.) Does not have health insurance (Uninsured), or c.) Is an American Indian, or
d.) Is an Alaskan Native, or
e.) Is a patient who receives benefits from Children's Health Insurance Plan (CHIP) *
f.) Is underinsured: 1) has commercial (private) health insurance, but coverage does not include vaccines; or 2) insurance covers only selected vaccines (TVFC-eligible for non-covered vaccines only).
Those children who are on Medicaid and CHIP will need to present their cards at the time of service and there is no charge for the vaccine. Those who do not have any form of insurance will be assessed a $10.00 administration fee per vaccine at the time of service. This payment can be in the form of a check (check number greater than 500) or cash. We do carry private stock vaccines that can be given to children (11years old and older) with insurance (Tdap, Varicella, MCV4) The only insurances that we file on are Blue Cross Blue Shield, Cigna and United Health Care. If having to pay for the private vaccine out of pocket, the cost is $50.00 for Tdap, $120.00 for Varicella and $120.00 for MCV4.
** NOTE: You must call before the clinic day if you have insurance so that it can be verified prior to the clinic time. We Will Not Be Able To Verify Insurance On The Clinic Day!
